Agape Missions guiding principles embraces compassionate and confidential services with respect for all lifestyles.
Case managers assist HIV+ clients with sources, information and referrals if needed to programs and services that may be needed. Case managers meet with clients in the office, at their homes, and maintain telephone contact in support of the individual service plans that have been developed. This partnership helps empower clients to help themselves to live more productive lives.
Through the AIDS Foundation of Chicago Cooperative, Case Management is defined as a standardized process of assessment, case planning, service coordination, referral, advocacy, and follow up through which the multiple service needs of persons affected by HIV/AIDS are met.
Services include: case management, housing programs, counseling, testing, prevention programs, recreational programs, education and outreach services to diverse populations in Will, Grundy, Kankakee, Southern Cook and surrounding counties.
