Description: Initial Service Coordination is from the time a family or agency contacts us about a developmental concern for a child under the age of three years through the eligibility determination and into services appropriate for the child.

Description: If you are concerned about your child crawling, walking, talking, understanding, resonding, seeing or hearing, your family may benefit from early intervention services. Child and Family Connections provides assistance for your family with receiving evaluations and assessments of your child, determining eligibility for early intervention services, developing an Individual Family Service Plan, providing parent support services, and linking you to the services you choose for your child.
